I found these records in Notre Dame (church) Montreal and I have transcribed them.

In the presence of the undersigned notaries have appeared Pierre Beaubien and Pierre Munro, Esq., physicians of Montreal, who have declared that the list above and on the previous page is correct and truly transcribed from the diary kept by them at the Cholera Hospital at Plaine Ste. Anne, in 1834.
Accordingly, they have signed a Montreal, this 12th of January one thousand eight hundred and forty three, with the said Notaries.
En pr?sence des Notaires soussign?s sont comparus Pierre Beaubien et Pierre Munro, Ecuiers, M?decins de Montr?al, qui ont declar? que la liste cidessus et ?s page pr?c?dente, est correcte et fid?liment extradite du Journal tenu par eux ? L?Hopital des Chol?riques, a la Plaine Ste. Anne, en 1834.
En foi de quoi, ils ont sign? ? Montreal ce douze Janvier nil huit cent quarante trois, avec les dits Notaires.
